I am working on a page the allows the user, once logged in, to upload
information and a picture. The picture is saved as his/her username, and if
they upload a different picture, the old one is still displayed unless they
leave the site and come back 10+ minutes later.

To send data I would like to create 5 input fields, and then use a submit
