Newbie having problem with form

Hello I am very new at flash but have managed to put form inputs on my
webpage..  you can view here www.puppypalacedaycare.com/bookparty.html   I am
wanting my clients to enter their information then click submit and send the
information to me via email. I know the coding in html but not sure what arrays
i need to make my submit button work and include all the information they
entered to be sent via email. I hope i am asking the right questions. I have
had flash for sometime now and still aint figured it out.. *sigh*   any help
would be greatly appreciated   Tanya
