cf_ThreeSelectsRelated custom control problem

I've used the cf_ThreeSelectsRelated custom control and I'm having a problem. I
need to use three of the cf_ThreeSelectsRelated controls on one form and it
doesn't work. Has anyone had and solved this problem?

You will need to duplicate the code (functions) once for each time you need
to use it giving each one a unique name.
